I’m Looking to Stop My GLP-1, Now What?

As a physician, I’ve seen firsthand the incredible impact that GLP-1 medications (like Semaglutide and Tirzepatide) can have on helping patients achieve significant weight loss and improve their cardiometabolic health. However, many patients eventually look to transition away from these medications for various reasons.

If you’re considering stopping your GLP-1 program, it’s a critical time to be proactive. The unfortunate reality is that studies show individuals can regain up to two-thirds  or more of the weight lost following cessation, and previous health improvements can reverse. Many health plans are shifting away from covering these medications for the sole purpose of weight management. The resulting out-of-pocket costs are often not feasible or sustainable long-term for most patients.

  • Significant Side Effects: While we employ physician-supervised strategies to mitigate them (like slow, customized titration), some patients find that the gastrointestinal side effects (nausea, vomiting, diarrhea, constipation, reflux) outweigh the benefits.

  • Reaching Goal: Although less common and generally not advised due to the risk of weight regain, some patients may reach their target weight or achieve significant health improvements and seek a transition plan.

The process of stopping your medication is just as important as the lifestyle changes you implement. Always discuss your dosage plan with your physician to create a safe, individualized schedule.

  • Avoid “Cold Turkey”: Simply quitting cold turkey often leads to a rapid return of intense hunger and cravings.

  • Consider Tapering: For most patients, weaning slowly seems to go better. This involves gradually reducing the dose over several weeks.

  • Maintenance Dosing: Given the known risk of weight regain, many patients find it easiest and most effective to continue the medication at a significantly reduced maintenance dose to help manage underlying hunger and hormonal signals long term.

  • The Easiest Option: Most patients find it easiest and most effective to continue a low-dose medication long term to maintain the health benefits and weight loss.

  • Consume More Fiber: Fiber supports digestion, steadies blood sugar, and promotes a prolonged feeling of fullness. Aim for at least 25g of fiber per day from whole foods.

  • Strategic Eating: I recommend three meals and one to two snacks spaced about 3–4 hours apart. This prevents intense hunger, which can lead to poor choices.

2. 💧 Encourage Aggressive Hydration

Your body can sometimes confuse thirst with hunger. Drink at least 64 oz of water or unsweetened beverages daily, and use alternatives like fruit-infused water or unsweetened tea if you don’t prefer plain water.

3. 💪 Emphasize Continued Exercise, Including Strength Training

The weight loss achieved on GLP-1s can sometimes include lean muscle mass loss. It is imperative to counteract this for long-term metabolic health.

  • Integrate Strength Training: At least 25%–50% of your weekly exercise should be strength training to maintain muscle mass, improve bone density, and support weight maintenance.

  • Follow Guidelines: Aim for 150 minutes per week of physical activity, primarily focusing on consistency.

4. 😴 Get Restful Sleep

Lack of restful sleep is a significant driver of overeating and poor food choices, as it negatively impacts hormones like ghrelin and leptin. Aim for 6–8 hours of high-quality rest per night in a cool, dark room.

5. 🧘 Manage Your Mindset and Stress

Unhelpful self-talk and high stress (elevated cortisol) can sabotage your best efforts and drive emotional eating. Utilize friends, family, community, and professional resources to help manage stress and build supportive thought patterns.
